Provide carrier name, consignee name, street address, city and state, commodity description, pieces, weight and classification on the BOL. After pickup, email or fax the BOL to Koch Logistics. Contact the Koch Logistics Team for 3rd Party Koch Bills of Lading or labels, and ensure EVEREVE billing account number is included in the purchase order data.
